File consists of pledge forms from medical practitioners in the County of Wentworth (Hamilton) in response the Order-in-Council of the Government of Ontario cancellation of medical relief for doctors to be paid for their service for patients who receive receipt of relief. There is a facsmilie of an invitation to attend a meeting of general practitioners where the original document was destroyed. Also included is a signed application to form the Section of General Practitioners within the Academy.

The Section of General Practice, the precursor to the Section of Family Medicine, was organized in response to the Welfare Board and the Committee of the City Council refusal to apply Hamilton the provisions of the Order-in-Council regarding medical relief, when many general practitioners felt that they were treated unfairly.
